Tamsulosin is a selective alpha-1A antagonist that relaxes urethral + prostatic smooth muscle. Used for functional urethral obstruction (urethral spasm), post-catheter recovery, BPH adjunct.
First-dose hypotension caution. AVOID with sildenafil (severe hypotension). Senior dogs start low.

Tamsulosin for Dogs – Urethral Relaxant

Tamsulosin (Flomax) is a selective alpha-1A antagonist that relaxes urethral + prostatic smooth muscle.

0.01-0.02 mg/kg PO every 12-24 hours

Indications

  • Functional urethral obstruction (urethral spasm)
  • Post-urinary obstruction recovery (7-14 days post-catheter)
  • Benign prostatic hyperplasia (adjunct – castration first-line)
  • Reflex dyssynergia
  • Detrusor-sphincter dyssynergia

Post-Urinary Obstruction Protocol

  1. Place urinary catheter to relieve obstruction
  2. Treat underlying cause (stones, plugs, idiopathic)
  3. After catheter removal: tamsulosin 0.01-0.02 mg/kg q12h x 7-14 days
  4. Monitor for re-obstruction
  5. Adjust based on response

⚠ Drug Interactions

AVOID

  • Sildenafil/tadalafil (PDE5 inhibitors) – severe hypotension
  • Nitrates – severe hypotension

Caution (Additive Hypotension)

  • ACE inhibitors
  • Calcium channel blockers
  • Beta-blockers

Other

  • Cimetidine – increases levels

Side Effects

  • First-dose hypotension (start LOW, observe)
  • Dizziness/weakness
  • Syncope (rare)
  • Compensatory tachycardia
  • Decreased appetite

Prazosin Alternative

Non-selective alpha blocker – common veterinary alternative:

  • 1 mg per dog small
  • 2 mg per dog larger
  • Q8-12H dosing
  • Lower cost
  • More available
  • More systemic hypotension

Administration

  • Empty stomach 30 min before food
  • Cannot crush ER capsules
  • Start LOW + titrate
  • Watch first-dose hypotension

How much tamsulosin for my dog?

DOSE: 0.01-0.02 mg/kg PO every 12-24 hours

EXAMPLES:

  1. 5 kg = 0.05-0.1 mg (compounded)
  2. 10 kg = 0.1-0.2 mg
  3. 20 kg = 0.2-0.4 mg
  4. 30 kg = 0.3-0.4 mg

CAPSULES: 0.4 mg ER (most common, Flomax); 0.2 mg compounded for smaller dogs. EMPTY STOMACH 30 minutes before food. Cannot crush ER capsules (modified release). Start LOW dose – first-dose hypotension risk. Senior dogs especially cautious. Watch for dizziness/weakness/collapse after first dose

PRAZOSIN alternative (non-selective alpha blocker, more available): 1 mg per dog small or 2 mg per dog larger q8-12h. WORK WITH VETERINARIAN for appropriate protocol

What is tamsulosin used for in dogs?

Primary uses for URETHRAL RELAXATION: 1) FUNCTIONAL URETHRAL OBSTRUCTION – urethral spasm after catheter removal (post-urethral plug, post-stone removal); 2) POST-URINARY OBSTRUCTION recovery – 7-14 days while urethral inflammation/spasm resolves; 3) BENIGN PROSTATIC HYPERPLASIA (BPH) in intact males – adjunct to castration (first-line); 4) Reflex dyssynergia – urethral muscle contracting inappropriately during voiding; 5) Detrusor-sphincter dyssynergia

MECHANISM:

  1. Selective alpha-1A adrenergic receptor antagonist
  2. Blocks receptors in urethra + prostate (not blood vessels selectively)
  3. Smooth muscle relaxation
  4. Improved urinary flow
  5. Less systemic hypotension than non-selective alpha blockers

WHEN APPROPRIATE:

  1. Recurrent urinary obstruction
  2. Difficulty initiating urination
  3. Stranguria (straining)
  4. Reduced urine stream
  5. Post-obstructive recovery

WHEN NOT APPROPRIATE:

  1. Mechanical obstruction (stones still present – need removal)
  2. Bladder atony (different problem)
  3. UTI primary (treat infection)
  4. Severe hypotension
  5. Concurrent sildenafil/nitrates

Can I give tamsulosin and sildenafil together?

AVOID combination – severe hypotension risk. INTERACTION:

  1. Both lower blood pressure
  2. Different mechanisms but additive effect
  3. Severe hypotension + syncope possible
  4. Particularly in senior dogs or those with cardiac disease

SAFER APPROACHES:

  1. Separate medications when both needed
  2. Use lowest doses with close monitoring
  3. Specialist consultation (cardiology + urology)
  4. Alternative for one indication if possible

WHEN BOTH MIGHT BE NEEDED:

  1. Pulmonary hypertension (sildenafil) + functional urethral obstruction (tamsulosin) – rare combination
  2. Cardiac specialist consultation essential
  3. ECG + BP monitoring
  4. Start each at lowest dose
  5. Owner observation

ALTERNATIVES:

  1. PRAZOSIN instead of tamsulosin (less hypotension typically)
  2. BETHANECHOL different mechanism (cholinergic) for some urinary issues
  3. Address sildenafil indication differently if possible
  4. Adjust timing – separate by hours

EMERGENCY signs if combined:

  1. Syncope (fainting)
  2. Severe weakness
  3. Collapse
  4. Mucous membrane pallor
  5. Bradycardia
  6. Emergency vet visit. Communicate ALL medications with veterinarian to avoid interactions
